Risk Factors Predictiong Gangrenous Change in Childhood Intussusception


Abstract
Advance of disease progress of intussusception causes necrosis of bowel and increases morbidity and mortality in pediatric ages. In order to identify risk factors predicting gangrenous change in intussusception, retrospective analysis was
performed
on
275 cases of intussusception in infancy and childhood who were admitted to the surgical and pediatric departments of Chonnam University Hospital from January 1980 to December 1989. Among the 275 patients of intussusception, 59% were treated with
barium
reduction, 33% with manual reduction and 8% with resection.
@ES The summeries were as follows:
@EN 1) Intussusception in younger age group was unlikely to be reduced by hydrostatic barium enema and increased risks requiring operation, but intussusception in older age group did not necessarily have increased risks of having pathologic
lead-point
or bowel necrosis.
2) Lethalgy and abdominal distention correlated considerably with severity of intussusception.
3) The most important risk factors statistically identified were duration of illness and small bowel obstruction on plain abdomen.
4) The presence of fever, leukocytosis and anemia did not give any information about prognostic significance.
5) The ileoileal intussusception were 31% of patients who required operative intervention, 59% of required resection.
6) Recurrent intussusception did not have increased risks of having bowel necrosis.
